Once you have spoken to us to get key advice and information from our Welfare Advisor we will ask you to complete an application form and you will need to get a supporting letter from your doctor. It is important that you do contact us before completing any paperwork please. During the processing of your application we may also ask that you attend an Occupational Therapy assessment at the Independent Living Centre, Semington.
